Q: Better aesthetically & less tired look via transconj. mid-face lift, cheek & tear trough implants, for 40-year old?  I was suggested the above + upper bleph, buccal fat pad removal & chin kybella. What type/position chick implant & how to decide? I need to have an (informed) vote in something this big, and to assume full responsibility for my decisions. Avoid tear trough implants even as long-term vs. yearly bruising & $$? Is kybella more efficient than chin lipo? Will buccal fat pad loss accentuate nasal/marionette lines? Will this all make a substantial improvement? I like my nose but open to tip narrowing.

A:  With each day of aging, skin, fat, fibrous tissues and bone changes will occur, The visual appearance can be enhanced for a longer period of time and more dramatically or a shorter period of time and subtly BUT aging marches on. 

Based on your photos, you have a stronger jawline width and narrower cheek width. The desired youthful appearance is a strong jawline but wider stronger cheek width and anterior projection, ie. what I call a nice cheek “apple” and “nike whoosh” extending laterally. This will highlight your cheeks and provide a models sub-malar hollow.

·         Consider fillers such as Radiesse along the bone of the cheeks for a greater foundation and HA (hyaluronic acid) fillers for the “pillow” fullness you desire. 

·         Cheek implants are definitely a consideration for you but there is a higher degree of dissatisfaction and complications.

·         A mid-face lift is an option but be aware of the side-effects and complications of this procedure too.

·         I would not recommend buccal fat excision as a means to obtain the above as the long term result has been too much hollowing. The thinner your face, the more draping that occurs. The more shadows in that draping, the older one looks. 

Regarding your neck, I am not a proponent of Kybella due to its inflammatory response but have used it. You may consider:

·         lipocontouring,

·         ThermiTIGHT,

·         Sculptra.

This is NOT advice. These are options which require the evaluation and recommendations of a skillful and artsy, technically competent Board Certified Plastic Surgeon to understand and if possible achieve your  desired results with the skeletal foundations of your unique face and neck.
