Q. Partial facelift or fillers? I’m a 61 year old male, had consultation with two surgeons regarding best option for me. First Dr., recommended surgery from neck up to below the eyes, he does not like to do any surgery from below the eyes up on men. Other recommended filler or fat injections. A series of 3 injections 6 month intervals. He stated that a facelift on men does not last beyond 1.5 years, elasticity is not same as a woman. HELP

A. Thanks for sharing your photos.  They show significant laxity and drooping of the skin and soft tissues.

The beginnings of facial skin laxity start when skin repair diminishes and passive skin aging relaxes downward. The skin stretches and elongates developing “waves” from the brows to the neck which need to lifted and excess removed and / or filled. If you can “pinch an inch” or more skin along the jawline, there is nothing better than a face lift result.

Volume loss and / or drooping redistribution of soft tissues also contribute to the marionette folds, jowls and boxy aging facial appearance. 

Since I do not have the opportunity to examine you, I hope you will be helped by the following information:

  • Consult with a Board Certified Plastic Surgeon with facial enhancement experience to examine, identify and manage your desires.
  • I believe you will be best served with a Face and Neck Lift. This will provide the longest lasting (average 7 years) option and properly redistribute the fat and soft tissue (like the jowls and cheeks) under the skin and redrape and remove excess skin in the most youthful approach.
  • While fillers, botox, suspension sutures, lasers and other non-invasive and minimally invasive options exist, they will only address portions of your concern, for far less duration and more total expense.

Everyone needs one or more layered improvement and in personalized combinations to achieve their optimal result. You can understand why it takes a talented individual, preferably a Board Certified Plastic Surgeon with great experience to evaluate your needs and find a plan that works best for you.
